New York Congregational Nursing Center  announced the opening of the Long Term Home Health Care Program, designed to provide professional care and services to 100 persons who would otherwise qualify for nursing home placement, but who elect to receive services while remaining in their homes.

 The LTHHCP represents a significant leap forward in the expansion of New York Congregational services and a major step in simultaneously meeting an identified need in our community. said Ms. Celia Zuckerman, President and CEO.  A full array of services is available to patients based upon a Professional Nurses assessment of their needs.  Services include Nursing, Rehab Therapy, Social Work, Aide Assistance and Nutritional Counseling.  Audiology and Respiratory Therapy Services, as well as Transportation, Medical Supplies and Equipment are also available.

 

Patients who would benefit from the new Long Term Home Health Care Program are those with multiple chronic conditions requiring case management; those who have difficulty following medication and treatment plans, and those who experience frequent hospitalizations.

  In order to be eligible for the LTHHCP patients must require at least two (2) services which the program provides, must have active Medicaid or be eligible for Medicaid coverage, and must reside in Brooklyn.
